how does that work when google play is a streaming service? the 3 months free is for streaming not downloads. do they save it to a device to listen to it offline then copy the file somehow?

Ofc, you adding file like to stream them offline on your device later, but then you just extract those files to your phone with playmusicexporter (at least this method worked for older androids, not sure about new, but it's almost same story i think) so they (mp3 files) are legit, without any watermark.
